Vital Records (Birth/Death/Marriage)

The Waitsfield Town Clerk’s office contains birth, death and marriage certificates. See the Town Clerk Fee Schedule for the current cost of certified copies. We can issue certified copies of birth and death certificates for anyone in the State upon completion of the Application for Certified Copy (PDF).  If you are mailing the application, please include a photocopy of your driver’s license or other form of identification as indicated on the application. We can provide copies of marriage licenses if you obtained your marriage license from our office.

Certified copies of marriage certificates can only be obtained from the Town where the license was issued.  We recommend calling our office at 802-496-2218 to confirm the license is here before moving forward.  Copies can be issued by coming into our office during regular hours or by mailing this Request Form to our office.
